Overview

Epilepsy is a chronic neurological disorder characterized by recurrent, unprovoked seizures resulting from abnormal neuronal activity in the brain. It affects approximately 50 million people globally and is one of the most common neurological disorders, impacting individuals of all ages but with varying prevalence across different demographics 123. The condition significantly impacts quality of life, cognitive function, and overall health outcomes. Effective management is crucial in day-to-day practice to control seizures, minimize comorbidities, and improve patient well-being 45.

Pathophysiology

The pathophysiology of epilepsy involves complex interactions at molecular, cellular, and network levels. At its core, epilepsy arises from an imbalance in excitatory and inhibitory neurotransmission within neural circuits. Genetic mutations, acquired brain injuries, infections, and developmental abnormalities can disrupt ion channel function, leading to hyperexcitability or hypoexcitability of neurons 67. This disruption often involves alterations in sodium, potassium, and calcium channels, which regulate neuronal firing patterns. Additionally, changes in neurotransmitter systems, particularly GABAergic inhibition and glutamatergic excitation, contribute to seizure generation 89. At the network level, these cellular abnormalities can lead to the formation of epileptogenic foci, where abnormal synchronous activity propagates, causing clinical seizures 1011.

Epidemiology

Epilepsy affects approximately 1% of the global population, with a lifetime prevalence ranging from 2.9 to 10.7 per 1000 individuals, depending on geographic location and diagnostic criteria 112. The incidence is highest in early childhood and older adulthood, with peak onset in early infancy and late adulthood. Males are slightly more likely to develop epilepsy than females, although this gender difference varies by age group and specific epilepsy syndromes 1314. Risk factors include perinatal injuries, infections (e.g., meningitis, encephalitis), brain tumors, and genetic predispositions. Over time, there has been a trend towards better diagnostic tools and increased awareness, potentially influencing reported prevalence rates 1516.

Clinical Presentation

Epileptic seizures manifest through a spectrum of motor, sensory, autonomic, and cognitive symptoms. Typical presentations include tonic-clonic (grand mal) seizures with convulsions, loss of consciousness, and postictal confusion; absence (petit mal) seizures characterized by brief staring spells and subtle motor signs; and focal seizures with motor or sensory features localized to specific body parts 1718. Red-flag features include status epilepticus (prolonged seizures lasting more than 5 minutes), atypical seizure patterns, and seizures associated with significant neurological deficits or sudden unexplained cognitive decline, which warrant urgent evaluation 1920.

Diagnosis

The diagnostic approach to epilepsy involves a comprehensive clinical evaluation, detailed patient history, and targeted diagnostic tests. Key steps include:

  • Detailed History: Comprehensive seizure description, including semiology, frequency, duration, and triggers.
  • Physical Examination: Neurological assessment to identify focal deficits or signs of underlying neurological conditions.
  • Electroencephalogram (EEG): Essential for confirming epileptiform activity; interictal epileptiform discharges are indicative 2122.
  • Imaging Studies: MRI or CT scans to rule out structural causes such as tumors, vascular malformations, or previous injuries 2324.
  • Laboratory Tests: Blood tests to exclude metabolic disorders, infections, and other systemic causes 2526.
  • Specific Criteria and Tests:

  • EEG Findings: Interictal epileptiform discharges (IEDs) consistent with focal or generalized epilepsy.
  • MRI/CT Criteria: Normal imaging rules out structural causes; abnormalities suggest secondary epilepsy.
  • Blood Tests: CBC, electrolytes, glucose, liver function tests, thyroid function tests, and toxicology screens as indicated 2728.
  • Differential Diagnosis:

  • Syncope: Episodes of transient loss of consciousness without convulsions or postictal state.
  • Psychogenic Non-Epileptic Seizures (PNES): Behavioral events mimicking seizures but lacking EEG correlates and often linked to psychological stressors 2930.
  • Migraines: Complex aura symptoms can mimic focal seizures but lack the characteristic EEG changes 3132.
  • Management

    First-Line Treatment

  • Antiepileptic Drugs (AEDs): Monotherapy is preferred initially. Common first-line options include:
  • - Carbamazepine: 10-20 mg/kg/day, divided twice daily 3334. - Lamotrigine: 5-25 mg/kg/day, titrated slowly to avoid hypersensitivity reactions 3536. - Levetiracetam: 20-60 mg/kg/day, divided twice daily 3738. - Valproate: 5-10 mg/kg/day, titrated carefully due to teratogenic risks 3940.

    Second-Line Treatment

  • Adjunctive Therapy: If monotherapy fails, consider adding another AED:
  • - Topiramate: 25-50 mg/day, titrated up to 50-100 mg/day 4142. - Gabapentin: 900-3600 mg/day, divided three times daily 4344. - Zonisamide: 100-300 mg/day, titrated gradually 4546.

    Refractory Epilepsy

  • Surgical Interventions: For medically refractory cases:
  • - Corpus Callosotomy: Palliative surgery for generalized seizures 147. - Temporal Lobectomy: For unilateral focal epilepsy 4849. - Deep Brain Stimulation (DBS): Targeting structures like the centromedian nucleus for multifocal/generalized epilepsy 250.
  • Neuromodulation: Vagus Nerve Stimulation (VNS) for refractory cases 551.
  • Ketogenic Diet: For patients intolerant to medications, especially children 352.
  • Contraindications:

  • Pregnancy: Avoid valproate; consider alternative AEDs with safer profiles 5354.
  • Renal/Hepatic Impairment: Adjust dosing based on organ function 5556.
  • Complications

    Acute Complications

  • Status Epilepticus (SE): Prolonged seizures requiring immediate intervention with benzodiazepines and potentially intubation 5758.
  • Injury: Falls, trauma, and aspiration during seizures 5960.
  • Long-Term Complications

  • Cognitive Decline: Chronic epilepsy can lead to progressive cognitive impairment 6162.
  • Cardiac Arrhythmias: Increased risk of SUDEP (Sudden Unexpected Death in Epilepsy) due to seizure-induced arrhythmias 6364.
  • Psychiatric Comorbidities: Higher incidence of depression, anxiety, and PTSD 6566.
  • Management Triggers

  • Regular Monitoring: Frequent follow-ups to adjust medications and assess cognitive function.
  • Emergency Protocols: Clear guidelines for managing SE and recognizing signs of SUDEP 6768.
  • Prognosis & Follow-Up

The prognosis for epilepsy varies widely depending on the etiology, seizure type, and response to treatment. Factors predicting better outcomes include early diagnosis, effective initial treatment, and absence of underlying neurological deficits 6970. Regular follow-up intervals typically include:
  • Initial Phase: Monthly visits for the first 6 months post-diagnosis.
  • Stabilization: Every 3-6 months if stable on treatment.
  • Long-Term: Annual evaluations to monitor medication efficacy, side effects, and cognitive health 7172.
  • Special Populations

    Pregnancy

  • Medication Choice: Avoid valproate; consider lamotrigine, levetiracetam, or carbamazepine 5373.
  • Monitoring: Frequent fetal ultrasounds and maternal serum alpha-fetoprotein levels 7475.
  • Pediatrics

  • Ketogenic Diet: Effective for drug-resistant epilepsy in children 7677.
  • Developmental Monitoring: Regular assessments to track cognitive and motor development 7879.
  • Elderly

  • Polypharmacy: Increased risk of drug interactions; careful medication review 680.
  • Comorbidities: Focus on managing cardiovascular and cognitive comorbidities 8182.
