Infection by Encephalitozoon cuniculi

Clinical Presentation

An HIV-negative lung transplant recipient presented with fever, lung infiltrate, and acute kidney injury, indicative of disseminated E. cuniculi infection [PMID:23388129].

Diagnosis

Renal biopsy revealed granulomatous interstitial nephritis with characteristic 'belt-like stripe' organisms in tubular epithelial cells, confirmed by electron microscopy, PCR, and cell culture [PMID:23388129].

Management

The patient was treated with albendazole and had immunosuppressive therapy reduced, though clinical outcomes were complicated by other infections [PMID:23388129].
