Laryngotracheobronchitis

Last edited: 6 h ago

Overview

Laryngotracheobronchitis (LTB), commonly known as croup, is an acute viral infection characterized by inflammation of the upper airway, leading to characteristic barking cough and stridor 1.

Diagnosis

  • Clinical presentation: Barking cough, inspiratory stridor, and retractions 1.
  • Arterial blood gas analysis: Elevated paCO2 (>37 mmHg) or decreased paO2 (<50 mmHg) indicative of respiratory compromise 1.
  • Vital signs: Tachycardia (pulse rate >170/min) and tachypnea (respiratory rate >55/min) 1.
  • Management

  • Supportive care: Maintain airway patency, humidified air, and hydration 1.
  • Epinephrine: Nebulized or intramuscular (dose specifics not provided in abstract) for relief of upper airway obstruction 1.
  • Corticosteroids: Oral or nebulized (dose specifics not provided in abstract) to reduce airway inflammation 1.
  • Special Populations

  • Pediatrics: Increased vigilance in malnourished children, post-measles patients, and those with comorbidities like lower respiratory tract infections or cardiac involvement 1.
  • Complications: Urgent intubation required in presence of cyanosis, hypotonia, or altered consciousness 1.
  • Key Recommendations

  • Intubation is urgently indicated in the presence of cyanosis, hypotonia, or altered consciousness 1 (Evidence: Strong).
  • Consider tracheal intubation if two or more of the following criteria are met: pulse rate >170/min, respiratory rate >55/min, paCO2 >37 mmHg, paO2 <50 mmHg, or presence of complications such as cardiac failure or severe lower respiratory tract infection 1 (Evidence: Moderate).
  • Closely monitor and manage children with underlying conditions like malnutrition, post-measles state, or cardiac involvement due to increased risk of severe airway obstruction 1 (Evidence: Expert opinion).
