Infertile

Last edited: 4/14/2026

Overview

Infertility refers to the inability to achieve pregnancy after a year of regular, unprotected intercourse, affecting approximately 10-15% of reproductive-aged couples. It can result from various factors including ovulatory disorders, tubal blockages, male factor issues, and unexplained causes. 4

Diagnosis

  • Medical History and Physical Examination: Essential for identifying potential causes such as menstrual irregularities, hormonal imbalances, and anatomical issues. 4
  • Ovulation Assessment: Utilize basal body temperature charts, ovulation predictor kits, or blood progesterone levels to confirm ovulation. 4
  • Hormonal Testing: Measure FSH, LH, estradiol, and prolactin levels to evaluate ovarian reserve and hormonal balance. 4
  • Semen Analysis: Evaluate sperm count, motility, and morphology in male partners. 4
  • Imaging Studies: Include hysterosalpingography or ultrasound to assess uterine and fallopian tube health. 4
  • Genetic Testing: Consider in cases with recurrent pregnancy loss or specific clinical presentations. 4
  • Management

  • Ovulation Induction: Use clomiphene citrate (50-150 mg/day) or letrozole (2.5-5 mg/day) for anovulatory disorders. 4
  • Intrauterine Insemination (IUI): Combine with ovulation induction for unexplained infertility or mild male factor issues. 4
  • Assisted Reproductive Technologies (ART): Include in vitro fertilization (IVF) for tubal factor infertility, severe male factor, or failed less invasive treatments. 4
  • Lifestyle Modifications: Recommend weight management, smoking cessation, and reduction of alcohol intake to improve fertility outcomes. 10
  • Psychological Support: Provide counseling to address emotional and psychological impacts of infertility treatments. 21
  • Preconception Care: Optimize nutritional status, particularly B-vitamin levels, to enhance reproductive outcomes. 10
  • Special Populations

  • Pregnancy Concerns: Evaluate occupational exposures that may impact reproductive health and consider medically necessary job modifications. 21
  • Pediatric Considerations: Limited direct evidence; focus on ensuring optimal hormonal and anatomical development in adolescents with concerns. 4
  • Elderly Patients: Address age-related hormonal changes and potential comorbidities affecting fertility, though ART success rates generally decline with age. 4
  • Comorbidities: Manage conditions like diabetes and thyroid disorders to optimize fertility outcomes, as these can significantly impact reproductive health. 13
  • Key Recommendations

  • Utilize Comprehensive Hormonal Assessments for diagnosing ovulatory disorders and hormonal imbalances in infertile couples. (Evidence: Strong 4)
  • Consider Preconception Optimization, including B-vitamin sufficiency, to mitigate adverse effects of environmental exposures like DDT on reproductive outcomes. (Evidence: Moderate 10)
  • Implement Psychological Support Services for patients undergoing infertility treatments to address emotional well-being. (Evidence: Expert opinion 21)
  • Evaluate and Address Occupational Exposures that may affect reproductive health, particularly in women concerned about workplace hazards. (Evidence: Moderate 21)
  • Tailor ART Approaches based on specific etiologies identified through diagnostic evaluations, prioritizing IVF for complex cases. (Evidence: Moderate 4)
