Right ventricular hypertrophy

Last edited: 4/22/2026

Overview

Right ventricular hypertrophy (RVH) is characterized by structural thickening and enlargement of the right ventricle in response to chronic pressure or volume overload, often secondary to conditions like pulmonary hypertension, chronic lung disease, or congenital heart defects. 1310

Diagnosis

  • Electrocardiogram (ECG) Findings: Increased R/S ratio in V1, deep S wave in V1, and tall R wave in V5 can suggest RVH, though specificity is limited. 121444
  • Echocardiography: Measures right ventricular wall thickness and dimensions; useful for diagnosing RVH with high sensitivity and specificity. 303743
  • Necropsy: Gold standard for confirming RVH; correlates well with echocardiographic measurements. 343
  • Thallium-201 Myocardial Scintigraphy: Can visualize RV free wall and assess RV overload quantitatively. 35
  • Management

  • Phosphodiesterase-5 Inhibitors: Sildenafil may improve RV function in pressure-overloaded hearts. 1
  • L-Type Calcium Channel Blockers: Agents like nitrendipine can prevent and treat RVH by modulating calcineurin signaling pathways. 2
  • Avoiding Coronary Ischemia: Monitor and manage coronary flow reserve, especially in severe RVH, to prevent ischemia. 203839
  • Sympathetic Modulation: Consider selective sympathetic neural changes in management strategies for RVH. 44
  • Special Populations

  • Pediatrics: ECG criteria for RVH in children require careful interpretation due to developmental differences. 12
  • Elderly: Age impacts the hypertrophic response to pressure overload; older individuals may have altered compensatory mechanisms. 16
  • Comorbidities: Conditions like chronic lung disease can exacerbate RVH; management should address underlying causes. 44
  • Key Recommendations

  • Use echocardiography for accurate diagnosis of RVH due to its high sensitivity and specificity. (Evidence: Strong 37)
  • Consider phosphodiesterase-5 inhibitors like sildenafil for patients with RVH secondary to pressure overload. (Evidence: Moderate 1)
  • Monitor coronary flow reserve in patients with severe RVH to prevent ischemia. (Evidence: Moderate 3839)
  • Tailor management strategies considering age-related differences in hypertrophic response. (Evidence: Expert opinion)
