Thyrotoxicosis

Last edited: 4/14/2026

Overview

Thyrotoxicosis refers to a clinical state characterized by elevated levels of thyroid hormones (T3 and T4) with or without suppressed TSH, often due to hyperthyroidism but also other causes like thyroid hormone resistance or exogenous intake 47.

Diagnosis

  • Clinical Presentation: Symptoms include weight loss, palpitations, tremor, anxiety, heat intolerance, and goiter 1314.
  • Laboratory Tests: Measure serum T3, T4, and TSH levels; low or suppressed TSH with elevated T3 and T4 confirms thyrotoxicosis 4.
  • Differential Diagnosis: Consider conditions mimicking thyrotoxicosis such as anxiety disorders, pheochromocytoma, and hypercalcemia 1323.
  • Special Tests: Radioiodine uptake and scan for differentiated thyroid disease; thyroid antibodies (TgAb, TPOAb) to identify autoimmune etiology 4.
  • Management

  • First-Line Treatments:
  • - Antithyroid Drugs: Methimazole (MMI) or propylthiouracil (PTU) for Graves' disease; dosing varies but typically MMI 10-40 mg/day or PTU 300-750 mg/day 14. - Radioiodine Therapy: Effective for definitive treatment, especially in non-pregnant adults; dose varies based on gland size 116.
  • Adjunctive Treatments:
  • - Beta-Blockers: For symptomatic relief (e.g., propranolol 10-30 mg TID) 127. - Thyroidectomy: Considered for refractory cases, large goiters, or when drug therapy is contraindicated 20.

    Special Populations

  • Pregnancy: Control thyrotoxicosis with propylthiouracil initially due to MMI's teratogenic risk; switch to carbimazole if needed; monitor closely 16.
  • Elderly: Atypical presentations common; careful evaluation to avoid misdiagnosis 14.
  • Comorbidities: Increased risk of atrial fibrillation and thromboembolic events; monitor closely in patients with cardiovascular disease 3142.
  • Key Recommendations

  • Accurate Diagnosis: Utilize TSH, T3, and T4 levels for diagnosis; consider thyroid antibodies and imaging for etiology 4 (Evidence: Strong).
  • Initial Treatment: Initiate antithyroid medications (MMI or PTU) based on patient factors; monitor closely for side effects 14 (Evidence: Strong).
  • Pregnancy Management: Use propylthiouracil initially in pregnant women with thyrotoxicosis; switch to carbimazole if necessary 16 (Evidence: Moderate).
  • Monitor Comorbidities: Closely monitor elderly patients and those with cardiovascular disease for complications like atrial fibrillation 3142 (Evidence: Moderate).
  • Consider Radioiodine: For definitive treatment in appropriate non-pregnant adults 116 (Evidence: Moderate).
