Chronic myeloid leukemia

Last edited: 4/13/2026

Overview

Chronic myeloid leukemia (CML) is a myeloproliferative neoplasm characterized by the BCR-ABL1 fusion gene, leading to uncontrolled proliferation of myeloid cells 5. It typically progresses through chronic, accelerated, and blast phases 2.

Diagnosis

  • Key Diagnostic Criteria: Presence of the Philadelphia chromosome (Ph) or BCR-ABL1 fusion gene 5.
  • Recommended Tests:
  • - Cytogenetic analysis for Ph chromosome 5. - Molecular testing for BCR-ABL1 transcript levels 5. - Complete blood count (CBC) showing elevated white blood cell count 33.
  • Grading:
  • - Based on cytogenetic response and molecular monitoring (e.g., major, minor, or no cytogenetic response) 233.

    Management

  • First-Line Treatments:
  • - Tyrosine Kinase Inhibitors (TKIs): Imatinib (400-800 mg/day), Dasatinib (100 mg/day), Nilotinib (300-400 mg/day), Ponatinib (15-45 mg/day) 234.
  • Adjunctive Treatments:
  • - Erythropoietic-stimulating agents for imatinib-induced anemia 31. - Management of comorbidities and polypharmacy in elderly patients 2729.
  • Treatment-Free Remission (TFR): Considered for patients with sustained deep molecular response (typically MR4.5) 1523.
  • Special Populations

  • Pregnancy:
  • - TKIs are generally contraindicated due to potential adverse events; alternative management strategies may be necessary 1.
  • Pediatrics:
  • - Imatinib is effective but requires monitoring for growth deceleration 8.
  • Elderly:
  • - Comorbidities and polypharmacy can impact treatment response and toxicity; individualized management is crucial 2729.
  • Comorbidities:
  • - Presence of conditions like IgA nephropathy may influence disease presentation 30.

    Key Recommendations

  • Initiate TKI therapy with imatinib, dasatinib, or nilotinib based on patient-specific factors and resistance profiles (Evidence: Strong 234).
  • Monitor molecular response using BCR-ABL1 transcript levels to guide treatment adjustments (Evidence: Strong 2).
  • Consider treatment-free remission for patients achieving sustained deep molecular response (MR4.5) (Evidence: Moderate 1523).
  • Carefully monitor pediatric patients for growth suppression while on TKI therapy (Evidence: Moderate 8).
  • Tailor treatment in elderly patients considering comorbidities and polypharmacy impacts (Evidence: Moderate 2729).
