Malignant otitis externa

Overview

Malignant otitis externa, also known as necrotising otitis externa, is a severe, invasive infection of the external auditory canal and surrounding structures, often caused by Pseudomonas aeruginosa, predominantly affecting immunocompromised individuals. 26

Diagnosis

  • Clinical Presentation: Severe otalgia, otorrhea, canal swelling, and potential cranial nerve involvement. 26
  • Imaging: Radiographic imaging may show soft tissue swelling or bone erosion in advanced cases. 26
  • Culture and Sensitivity: Essential for identifying the causative organism and guiding antibiotic therapy. 8
  • Grading: Often staged based on extent of canal and bone involvement, though standardized grading systems vary. 26
  • Management

  • Antibiotics: Initial broad-spectrum coverage followed by targeted therapy based on culture results, often including fluoroquinolones or ceftazidime. 268

  • Immunomodulatory Therapy: Consideration in immunocompromised patients, possibly including corticosteroids under specialist guidance. 26
