Ulnar neuritis

Last edited: 5 h ago

Overview

Ulnar neuritis involves inflammation or entrapment of the ulnar nerve, often leading to symptoms such as numbness, tingling, and weakness in the hand and forearm, particularly affecting the little finger and adjacent fingers. 1

Diagnosis

  • Clinical history and physical examination focusing on Tinel's sign and provocative tests (e.g., elbow flexion test).
  • Electromyography (EMG) and nerve conduction studies to assess nerve function and localize lesions.
  • Imaging (e.g., MRI) may be considered to rule out structural causes like fractures or tumors. 1
  • Management

  • First-line treatments: Conservative management including splinting, activity modification, and nonsteroidal anti-inflammatory drugs (NSAIDs) for pain and inflammation.
  • Adjunctive treatments: Corticosteroid injections into the cubital tunnel may be considered for refractory cases.
  • Physical therapy: Including modalities like ultrasound and exercises to improve nerve mobility and strength.
  • Surgical intervention: For chronic or severe cases unresponsive to conservative measures, decompression surgery of the ulnar nerve may be indicated. 1
  • Special Populations

  • Pregnancy: Limited data; conservative management is typically preferred due to risks associated with surgical interventions.
  • Elderly: Conservative approaches are often favored due to increased surgical risks; careful consideration of comorbidities is essential. 1
  • Key Recommendations

  • Initiate conservative management with splinting, activity modification, and NSAIDs for most patients with ulnar neuritis. (Evidence: Moderate) 1
  • Consider corticosteroid injections for patients with persistent symptoms despite conservative measures. (Evidence: Weak) 1
  • Evaluate surgical decompression for chronic cases that do not respond to non-operative treatments, especially in younger patients with significant functional impairment. (Evidence: Expert opinion) 1
