Acute alcoholic liver disease

Overview

Alcoholic liver disease (ALD) encompasses a spectrum of liver conditions from simple steatosis to severe alcoholic hepatitis, fibrosis, cirrhosis, and hepatocellular carcinoma, driven by both ethanol toxicity and nutritional deficiencies 1232229.

Diagnosis

  • Clinical history of significant alcohol use
  • Physical examination for signs of liver disease (jaundice, ascites, encephalopathy)
  • Laboratory tests: elevated liver enzymes (AST, ALT), hyperbilirubinemia, hypoalbuminemia, coagulopathy
  • Imaging: ultrasound, CT, MRI to assess liver morphology and detect complications
  • Liver biopsy (controversial; clinical diagnosis often sufficient 25)
  • Management

  • First-line treatments:
  • - Abstinence from alcohol: Essential for all stages 122 - Nutritional support: Addressing B-vitamin deficiencies, particularly folate, vitamins B6 and B12 5728
  • Adjunctive treatments:
  • - Corticosteroids: Considered in severe alcoholic hepatitis with Maddrey's score ≥32 26 - Lecithin with vitamin-B complex: Potential benefit in immunomodulation and oxidative stress reduction 911 - Prophylactic antibiotics: Amoxicillin-clavulanate may reduce mortality in severe cases 2 (Evidence: Moderate)

    Special Populations

  • Elderly: Higher mortality rates in ICU settings; careful monitoring and supportive care essential 6
  • Comorbidities: Systemic inflammation and multi-organ failure common in acute-on-chronic liver failure; intensive management required 4
